This kind of negative feedback system is common when it comes to how hormones work. It’s just like a thermostat and heater. As the room gets warmer, the thermostat sends less electricity to the heater. When the room gets colder, the thermostat sends more electricity to the heater.
Clomiphene works by blocking estrogen at the pituitary. The pituitary sees less estrogen, and makes more LH. More LH means that the Leydig cells in the testis make more testosterone.
As I explained in my last post, giving testosterone to a man does just the opposite. The pituitary thinks that the testis is making plenty of testosterone, and LH falls. As a result, the testis stops making testosterone, and the usually high levels of testosterone in the testis fall to the lower level in the blood.
So clomiphene is a way to increase testosterone in the blood and the testis at the same time. It preserves testis size and function while increasing blood testosterone.
Does Clomiphene citrate for men work?
Overall, the success rate for treating male infertility with drugs is lower than the success rate for women. For example, experts expect about 20-25% of men on fertility drugs to fertilize their partner. One study found that a combination of Clomid male infertility treatments and the antioxidant vitamin E boosted sperm count and mobility. And, ultimately, 37% of the partners of the men who took the combination became pregnant, while only 13% of partners with men taking a placebo became pregnant.

Clomid Success Rates for Male Infertility Treatment:
Success can be defined in two different ways: improvement in sperm health and testosterone levels, and in pregnancy rates.
What kind of pregnancy rate can you expect? Your personal situation will depend not only on your fertility, but also your partner’s fertility.
In studies including only couples facing male infertility, pregnancy rates after Clomid treatment vary, with some studies showing no improvement and others showing a fertility boost. The average pregnancy rate is around 13 to 15%.
A small but interesting study found that Clomid along with antioxidant treatment (like Vitamin-E) significantly raised pregnancy rates, up to 36%.
One meta-analysis looked at the results of several studies on Clomid and male infertility, specifically men experiencing idiopathic low sperm count and/or poor sperm motility.
This study found that treatment with Clomid, compared to no treatment, increased sperm concentration by about 5% and improved sperm motility by about 4%.
Reproductive hormone levels also significantly improved with Clomid treatment, with FSH levels rising 4% overall and testosterone levels rising 54%.
Perhaps the most important news for those trying to conceive, pregnancy rates doubled with Clomid treatment, when compared to those who received no treatment.
What about men experiencing azoospermia?
In one study, Clomid helped 64.3% of the men to produce sperm, with semen analysis results showing between 1 and 16 million sperm per milliliter.
For the men who did not produce sperm after Clomid treatment, all patients in this study had enough sperm to be retrieved using testicular sperm extraction. The extracted sperm could then be used for ICSI-IVF treatment.
Clomid was one of the original drugs used in post-cycle-therapy to stave off gynocomastia and raise the body’s natural testosterone levels. There are some side effects involved with heavy prolonged use, such as vision problems, and there are more effective substances on the market that do the same thing, but Clomid is still an effective and inexpensive compound for any athlete’s post-cycle-therapy.
